What Health System Pharmacy contributes to Cardinal Health
Health System Pharmacy is responsible for providing customized pharmacy program solutions that reduce costs and improve patient care quality for hospitals, health systems and other integrated healthcare providers may also consult with and advise healthcare team on prescribed medications, supplies and related processes.
The MTM Fulfillment Coordinator will provide assistance to the MTM Pharmacist to support our mission of delivering safe, effective and affordable medication therapy to our target population. The MTM Fulfillment Coordinator will assist the Pharmacist to help provide and ensure outstanding customer service. In addition, he/she will assume the responsibility of performing clerical duties, which do not require the professional clinical judgment of the Pharmacist.
Accountabilities:
Input demographic, insurance, medical and medication information into the MTM platform as needed
Contact patients to introduce and explain the benefits of the MTM program
Schedule appointments for the MTM Pharmacist to complete the Comprehensive Medication Review or the Targeted Intervention
Contact patients as needed for follow-up and adherence monitoring
Receive and respond to calls from provider/physician offices, patients and pharmacies; make determinations of appropriate action required from calls and escalate calls as needed
Obtain correct/updated health care provider contact information
Make outbound calls to providers and pharmacies to support MTM operations
Ensure each patient has a positive experience by providing a high level of customer service by answering promptly, professionally, and politely
Accurately document all service events in a timely manner
Verify appropriate information provided by patients with pharmacies, providers and laboratories
Coordinate outbound faxing and subsequent follow-up calls with health care providers
Obtain provider and patient contact information from designated resources to communicate medication reviews and recommendations
Be responsive to MTM Pharmacists regarding service fulfillment requests
Provide outstanding customer service
Perform auxiliary functions essential to the efficient operation of the department
Meet productivity and performance expectations set forth by manager
Skills Required to Perform Job:

Computer/Technology Competency
Proficient in use of Microsoft Word, Excel, and Outlook
Strong data entry skills (alpha and numeric)
Able to multi-task using multiple computer databases/platforms/application
Qualifications:
Preferred -
High School diploma or equivalent
Previous pharmacy technician experience
Available to work various shifts, including evenings and weekends when needed.
Previous medical background or experience in a health care setting
Experience with MTM Platforms, electronic health records and scheduling software
Bilingual: In addition to English, ideal applicant would also speak Spanish
Required -
Active pharmacy technician license in the state of Texas (PTCB: Pharmacy Technician Certification Board - Nationally Accredited Certification Exam)
Monday - Friday 8am - 7pm availability
Possesses knowledge of medication and medical terminology, including basic disease state knowledge
Ability to use and pronounce common medical and pharmacy terms correctly
Ability to access, use and understand pharmaceutical references
Possesses general knowledge of the therapeutic categories and common indications of medications
Ability to evaluate information from both oral and written sources, pharmaceutical references and report information to pharmacists
Understand and articulate the pharmacist's clinical role in the care of patients
Detail oriented, including ability to follow step-by-step instructions and enter data accurately into MTM database
Ability to provide excellent customer service with a can-do attitude
Proven record of excellent attendance and punctuality
Proven ability to work independently in a collaborative environment
